Abstract: 

یکی از ویژگی های بارز آموزش پزشکی، لزوم یادگیری مهارت های عملی و ارتباطی در کنار آموزش های نظری و دانش تئوریک می باشد. پرورش مهارت های بالینی، عنصر پایه آموزش پزشکی و بخش مهمی از برنامه های محوری کلیه، دانشکده های پزشکی را تشکیل می دهد. کلیه دانشجویان پزشکی می بایست مهارت هایی را که برای آینده حرفه ای خود به آن نیاز دارند مانند مهارت گرفتن شرح حال، معاینه فیزیکی، مهارت های ارتباطی، تشخیصی و درمانی، مهارت های مدیریتی در زمینه های بهداشتی درمانی، طبابت مبتنی بر شواهد، توانایی استدلال و تصمیم گیری بالینی، کار تیمی، توانایی سرپرستی تیم بهداشتی و نیز توانایی مراقبت از بیماران را به نحو احسن آموخته، در عمل به کار گیرند.

Abstract: 

Background: Coding errors lead to incorrect classification of diseases and thus severely affect health care planning and epidemiological research. Objectives: This study aimed to investigate the factors affecting Clinical Coding errors. Methods: This descriptive study was conducted in 2020 in 2 stages. In the first stage, a questionnaire was developed by examining relevant resources, documents, and articles. The questionnaire’, s validity was examined by 5 health information management experts. Its reliability was determined using Cronbach ,and correlation coefficient (r = 0. 84%). In the second stage, the data were collected by visiting teaching hospitals affiliated with Shahid Beheshti University of Medical Sciences, examining the medical records, and interviewing the coders. Data were analyzed using descriptive statistics. Results: Non-observance of diagnostic principles by physicians, illegibility of medical records, use of ambiguous and nonstandard abbreviations, and incomplete medical documentation were the most important causes of Coding errors. Conclusions: Considering the importance of Coding in presenting correct data as a powerful lever in health care, knowledge of the factors affecting the occurrence of Coding errors will greatly contribute to the selection of effective strategies to reduce and eliminate errors.

Abstract: 

Introduction: Every health organization life depends on its correct Coding system. One of the best tools in collecting correct and valid data is to make a Clinical Coding and procedures. Clinical Coding policy and procedures, in face of personals changes causes stability, and continues the Clinical Coding and provide a frame for decision making and doing duties. Methods: This research was carried out as a descriptive-comparative study. Information has taken from books, papers, internet, and health care institutes. The Coding policies and procedures from United State, England, Canada, and Iran; have been reviewed in this research. Due to similarity or differences in specifications, the suggested paradigm for Iran has been prepared. Then the model has been tested by Delphi method in two phases. And after evaluating it, the final model is presented. Results: Data showed that procedures and Coding policies have been presented in main six methods including: documentation and Coding principles, Coding information validating, Coding unit structure, Coding training, Coding communication, and security and privacy of Coding. Fourteen methods related to Coding policy are covered by Clinical Coding unit for these six axis. In each of these axis. Certain procedures have been achieved in each mentioned axis. Documentation principle, Coding procedures, time frame Coding, summarized Coding presentations, documentations and Coding error investigations, errors corrections and educational sections, employees job situations and numbers of employee that needed employees evaluations, in-services training, conditions that effected job, and tracking reviewing of recording data systems are some important procedures for these six axis. Conclusions: In total the final model for the Clinical Coding policies and procedures in Iran have more similarities by United State and England; and less by Canada. It have been suggested that Coding procedure would be evaluated and upgraded annually and have will be published in special magazine. To evaluate the Coding information and Coding procedures, an investigation committee have been suggested, in which based on their results, some workshops to be settled. Also for collection information from physicians about important cases, a special questioner form would be designed.

Abstract: 

Clinical Coding is a translation of medical documents to diagnostic, procedure and financial codes. Health care data are displayed with codes. Clinical Coding is a knowledge process in health care and include concepts of knowledge management such as codification of knowledge, storage of knowledge, packaging of knowledge with its details and delivery to users and coordinating of intellectual resources.Conversion process of data into knowledge is a continual process in health care industry and knowledge created in any phase is a data for upper phase too. So Clinical Coding performs organization of knowledge is related to diseases. The databases taken from Clinical Coding process display diagnostic data and codes in relational table formats of data mining and On-Line Analysis Processing (OLAP) can discover knowledge from it. Furthermore creation of On-Line Discussion Group can play important role in knowledge sharing and improvement in process of Health care information management particularly Clinical Coding.Health care information management professions need to identify real situation of Clinical Coding and impact of their functions on knowledge management in healthcare and health decisions and thus improve their practice with knowledge management tools.

Abstract: 

Introduction: The review of medical records with the aim of assessing the quality of codes has long been conducted in different countries. Auditing medical Coding, as an instructive approach, could help to review the quality of codes objectively using defined attributes, and this in turn would lead to improvement of the quality of codes.Method: The current study aimed to present a model for auditing the quality of Clinical codes. The audit model was formed after reviewing other audit models, considering their strengths and weaknesses. A clear definition was presented for each quality attribute and more detailed criteria were then set for assessing the quality of codes.Results: The audit tool (based on the quality attributes included legibility, relevancy, completeness, accuracy, definition and timeliness); led to development of an audit model for assessing the quality of medical Coding. Delphi technique was then used to reassure the validity of the model.Conclusion: The inclusive audit model designed could provide a reliable and valid basis for assessing the quality of codes considering more quality attributes and their clear definition. The inter-observer check suggested in the method of auditing is of particular importance to reassure the reliability of Coding.

Abstract: 

2Objective Augmented reality (AR) is one of the new technologies that is used in education, especially medical education. This study aims to determine the requirements for developing an educational application based on AR for teaching Clinical Coding. Methods In this descriptive-comparative study conducted in 2022, we first investigated the user interface (UI) and functionality of the AR-based educational applications. The UI features were extracted from 12 similar applications in Persian and English available in Google Play and Cafe Bazaar. To determine the functionality features, the opinions of a panel of experts including professors of Clinical Coding from Isfahan University of Medical Sciences in Iran were used.   Results The UI features included search (found in 3 applications), scanners (found in 11 applications), educational content classification (found in 9 applications), audio/text guides (found in 9 applications), and about us (found in 8 applications) sections. The functionality features included testing of knowledge and using images (2D, 3D). Conclusion Due to the theoretical and practical nature of the Clinical Coding course, a section to solve Clinical Coding scenarios is an important feature of AR-based educational applications for Clinical Coding. Also, the use of images (2D, 3D) in these applications can create a deeper impact on students’ learning.

Abstract: 

Background: During the COVID-19 pandemic, disease Coding significantly influenced national and international strategies for prevention, treatment, and control. This study aimed to assess the factors influencing COVID-19 records' Coding errors in teaching hospitals in South Khorasan province. Materials and Methods: In 2022, researchers conducted a cross-sectional study in South Khorasan Province, Iran. Data was gathered using a self-developed questionnaire distributed among medical coders in all hospitals. Descriptive statistics were employed in the data analysis using SPSS version 19. Results: The study's results showed that non-observance of diagnostic principles by physicians (66 out of 92), a lack of specialized medical Coding specialists (52 out of 92), and the use of ambiguous and non-standard abbreviations (51 out of 92) were the most significant factors affecting COVID-19 Coding errors. Among the three main factors affecting COVID-19 records' Coding errors, factors related to the medical coder (47.66 out of 92), factors related to healthcare providers (29.8 out of 92), and organizational and environmental factors (21.4 out of 92) were the most significant. Conclusion: This study revealed that multiple factors contribute to Coding errors in COVID-19 records, with the most critical being physicians' non-adherence to diagnostic principles, the shortage of medical Coding specialists, the use of unclear and non-standard abbreviations, and the absence of COVID-19 Coding continuing education. Therefore, we recommend conducting workshops on diagnosis and Coding for medical coders, particularly emphasizing the precise Coding of COVID-19 records.
